The attached file describes the options for the CSV file. The first two lines show the minimum example and an all supported fields example. Also keep in mind that although it's listed as CSV, it uses the Windows list separator, which may not be a comma for our International users. It is not suggested that you change your Windows setting as that may affect other programs on your machine, rather use the Windows separator in your CSV file.
